​I am a fee-for-service, out-of-network provider for most insurances.

As an out-of-network licensed provider, I can provide you with a superbill you can submit to your insurance company for reimbursement. Please note that insurance reimbursement is not guaranteed, and should be discussed with your insurance company in advance.

The fee is determined in advance and is payable at the time of the session.

I accept a variety of payment options for our services, including credit cards, cash, and checks at the time of the session.

I also accept Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) as payment.

I will be accepting Medicare Part B and Blue Cross Blue Shield (in-network) in late Fall 2023.

What should I ask my insurance company?
You can call your insurance company to inquire about reimbursement with a Superbill submission. Here are some questions that you can ask your insurance company if they will reimburse you for Medical Nutrition Therapy (MNT) coverage:

-Am I covered for MNT services?
-How many visits per year am I allowed? Is there a limit?
-Do I need a referral from PCP/DO or Specialist for services?
-Is there a specific diagnosis code covered?
-Do I need prior authorization before I have my appointment?
-Am I eligible for preventive nutrition appointments?
-Do I have any out-of-pocket expenses?

Below is helpful information to have when calling your insurance company.
The most common procedure code (CPT) for RDN is 97802 (initial visit) and 97803 (follow-up visit).
The ICD-10 code is established by your PCP in the referral for Medical Nutrition Therapy based conditions.
A common RDN ICD-10 code for Nutrition Services and Prevention is Z71.3

What should I bring with me to the appointment?
You do not have to bring anything to the first appointment. However, if you have the following available, it will be helpful and improve the evaluation.
-Blood or lab work (recent and past year)
-Medical history
-Any blood pressure, blood glucose readings
-Your payment
What if I have to cancel or reschedule my appointment?
Please provide 24 hours' notice when canceling or rescheduling an appointment. After your initial appointment and one cancellation/no-show, you will be charged a fee.
